Complete and submit the following 2 forms:

- Youth Advisory Committee Media Release Form

- Youth Advisory Committee Member Parental Consent, Waiver and Release of Liability

Both forms need to be completed by the youth and parent and/or legal guardian if youth is under the age of 18.
